The act of temporarily foregoing alcohol as part of a challenge frequently correlates with ongoing positive outcomes, including a reduction in alcohol consumption after the challenge concludes. This paper outlines three research priorities concerning TACs, as identified by our team. A rigorous assessment of the contribution of temporary abstinence itself, without the accompanying resources provided by TAC organizers (e.g., mobile applications and support groups), to alterations in consumption post-TAC is required. Subsequently, the psychological adaptations underlying changes in alcohol consumption remain elusive, with contradictory research on the role of enhanced personal conviction in not drinking as a mediator between involvement in a TAC program and subsequent reduction in consumption. Psychological and social pathways to change, while potentially significant, remain under-examined. Furthermore, evidence of higher consumption levels after TAC among a segment of participants indicates the imperative to pinpoint the circumstances or groups of people for whom TAC involvement may result in unfavorable outcomes. Research focused on these areas would significantly improve the confidence in facilitating participation. Long-term change would also be facilitated by prioritizing and tailoring campaign messaging and additional support to ensure maximum effectiveness.
The widespread prescribing of psychotropic medications, particularly antipsychotics, for behavioral difficulties in people with intellectual disabilities who are not psychiatrically ill, represents a significant public health concern. England's National Health Service launched the 'STopping Over-Medication of People with learning disabilities, autism or both (STOMP)' initiative in 2016, with the aim of confronting this issue in the United Kingdom. Rationalizing psychotropic medication use in individuals with intellectual disabilities is the anticipated outcome of STOMP's adoption by psychiatrists in the UK and beyond. By surveying UK psychiatrists, this research aims to understand their viewpoints and practical experiences related to the STOMP initiative implementation.
An online questionnaire was sent to each UK psychiatrist engaged in the work of intellectual disabilities (approximately 225 participants). By way of two open-ended questions, participants were afforded the opportunity to furnish feedback within the designated free text entry boxes. Locally, psychiatrists inquired about the obstacles they encountered in implementing STOMP, while another query sought illustrations of successful outcomes and positive experiences stemming from the process. Eighty-eight completed questionnaires were received from psychiatrists, accounting for approximately 39% of the total surveyed. In locations with robust STOMP support systems, psychiatrists reported contentment in the course of antipsychotic rationalization, an improvement in local multi-disciplinary and multi-agency collaboration, and heightened awareness of STOMP matters among stakeholders, encompassing individuals with intellectual disabilities and their caregivers, along with multidisciplinary teams; this also improved quality of life for individuals with intellectual disabilities by reducing the incidence of medication-related adverse effects. Despite optimal resource usage, in cases of suboptimal utilization, psychiatrists' satisfaction with the medication rationalization process was notably lacking, showing minimal improvements.
